REDUCTION
GYNAECOMASTIA
(SURGICAL REDUCTION OF ENLARGED MALE BREAST)
In
some boys during adolescence, breasts develop to a notable
size. This can cause psychological problems and causes
the boys to the embarrassed to remove their clothes
in front of others, like in the locker rooms or in the
swimming pools. But, the breasts become normal and almost
flat in most of these boys, in two or three years from
the time they had developed the breasts. In a few persons
the enlarged breasts persist, requiring surgical correction.
On rare occasions, breasts could develop in adult males
also, and sometimes it could be associated with hormonal
problems.
SURGICAL PROCEDURE:
In smaller gynaecomastia the surgery could be performed
under local anaesthesia without any hospital stay. But
most of the patients require general anaesthesia and
1 day stay in the hospital. To do both sides it takes
2 hours (longer if the breast are to be tightened at
the same time).
An incision (cut) is made at the margin of the lower
areola (the dark skin that surrounds the nipple). The
skin and the areola are lifted up and the breast tissue
with surrounding fat is removed. A suction drain is
placed before suturing the wounds.
For large gynaecomastia, either liposuction with excision
in done. Through a small stab incision in side of the
chest, a cannula is introduced and the fatty component
of the breast is suctioned out. This reduces the size
considerably. If the glandular component is still present
then it is excised through a separate incision made
at the margin of the lower areola (the dark skin that
surrounds the nipple). This procedure has reduced the
need for skin resection and tightening even in large
gynaecomastia.
